About 80% of Merkel cell carcinomas belong to the intermediate cell type with a very homogeneous cell pattern of medium-sized cells and rare stroma (fig. 2). In addition a small cell type exists which seems to have the poorest prognosis. Therefore, in routine histology the diagnosis is often difficult and, for years, it had to be proved by electron microscopy by demonstrating the typical neuroendocrine granules of the tumor cells [8].
